This is a screen-used foam latex prosthetic that was used in the filming of Jeepers Creepers 2. The prosthetic was attached to a full Creeper bodysuit worn by actor Jonathan Breck for his role reprisal of “The CREEPER”, and was obtained directly from a crew member.
